We provide hospice care for patients who have a life-limiting condition. Our comprehensive, inter-professional team includes physicians, skilled nurses, social workers, hospice aides, spiritual care counselors, bereavement counselors and volunteers who offer companionship and support, as well as other experts depending on their patient's needs. Working with the patient’s doctors to determine hospice eligibility, we develop a personalized care plan – based upon the patient’s needs and wishes – designed to maximize comfort and quality of life. Our focus is on the patient, 24 hours a day, 7 days a week. One of the major benefits of hospice care is the ongoing support we provide for each patient's family during the difficult days and nights following the patient's passing.
